On the upper control arm, the rear bushing has a sleeve that goes arond the bushing. Are there supposed to be one on each bushing or only the rearmost ones? If four are required where can I get them? Thank you for any help you can offer.

Nick, only one sleeve per arm is needed. This was an add-on instituted around 1970. If you're not doing a 100-point resto, an even better (and simpler, cheaper) method is two small "daubs" of mig weld on each bushing's outer shell (inner end), to prevent them from sliding in case the press fit is less than perfect.
